CHWs are the heart of CRUSH. From bedside to curbside, they guide clients through complex systems—hospitals, shelters, clinics, courts—with compassion and persistence.

Supporting Medi-Cal members transitioning from hospital to home with CHW-led care coordination.

Our integrated behavioral health team offers therapy, counseling, and recovery support grounded in real-life understanding and evidence-based practices.

We help individuals coming home from incarceration find stable ground—through housing navigation, workforce support, and peer mentorship.
